We are committed to providing you with the best ENT (ear, nose and throat) care services. To make sure we are delivering on our commitment, we want to make sure you are aware of the various costs associated with your care. To serve as many patients as we possibly can, our physicians participate in numerous insurance plans. The list below is provided as a helpful guide. As things constantly change, we suggest you check your own policy plan prior to your visit for the most up to-date insurance information.

When you provide us with your complete insurance information, we will submit your claims to your insurance carrier to receive payment for your services. Based on your insurance coverage, you may be responsible for pre-payments, co-payments, co-insurance, or other deductibles.
It is also your responsibility to confirm whether your insurance plan has restrictions on the doctors you can see and/or the services you can obtain. Please refer to your insurance policy plan details for information about exclusions.
If we are not contracted with your plan, you may have some level of coverage for out-of-network providers. If you don’t see your insurance carrier on the above list, please contact our offices or call your insurance carrier if you have any questions about the extent of your coverage.
